Please arrive 5-10 minutes early for your initial appointment to allow time for registration. Some patients will also be asked to complete a questionnaire.
During the first consultation, Dr Sved will obtain a detailed medical history. Most patients will also require a physical examination. Any relevant X-ray, ultrasound or CAT scans will also be reviewed.
Please ensure that you bring the following to your initial consultation
- A referral from your family doctor or other referring doctor (eg referring specialist)
- Any relevant X-Ray, ultrasound or CAT scans
- Copies of recent blood and urine test results
- A list of all your current medications
- Your Medicare card, DVA card or Pension card
- Details of your Private Health Insurance
If English is not your first language and you feel that you may not be able to give a detailed medical history in English, please ask an English-speaking family member or friend to accompany you to act as an interpreter.
If you need to cancel an appointment
Please telephone our office on 9747 5477 during business hours and allow at least 24 hours notice if possible so that your appointment time may be allocated to another patient.
We understand that your time is valuable and that you may have travelled a long distance to see Dr Sved. We therefore endeavour to keep to schedule. However, delays may occur if a prior patient has an unusually complex condition or if Dr Sved is called to perform emergency surgery or a Kidney Transplant. We sincerely apologise for any inconvenience this may cause.
